Services Agreement for Group Sessions

This form is designed to collect client information, consent, and electronic signatures for Group Session Participation Agreement (“Agreement”) governs your participation in group life consulting sessions (e.g., Empowerment Circle) provided by Beyond Genetics, a Florida-based company. By signing this Agreement, you agree to the terms below for personal development services offered virtually or in-person in Broward County, Florida.

Beyond Genetics Group Consulting Services Agreement

Welcome to Beyond Genetics! Please complete this form to agree to the terms of our group life consulting services, including consent for specific techniques. All fields are required unless marked optional. This form is for clients aged 18 and older only.

Beyond Genetics offers group life consulting sessions for adults (18+) to foster personal growth, using Neuro-Linguistic Programming (NLP), psychological insights, and optional techniques such as trance, hypnosis, kinesthetic methods (e.g., light contact on hand or shoulder for NLP purposes), or multisensory elements (e.g., bright/red light). Sessions are not medical, psychological, or therapeutic treatment, and Beyond Genetics is not a licensed therapist or medical professional.

Services Agreement Terms:

Beyond Genetic’s services are for personal development only. They do not diagnose or treat medical, psychological, or psychiatric conditions. Results are not guaranteed and depend on your active participation. Multiple sessions may be required. You acknowledge that sessions may evoke emotional responses and assume responsibility for your experience.

Beyond Genetics will keep your information, including the optional questionnaire, confidential indefinitely, except as required by law (e.g., court orders) or with your explicit written consent. You agree to respect the confidentiality of other group participants.

Fees are as specified at booking, payable via credit card, PayPal, or pay links prior to the session. Pricing is subject to change, and you will be notified at booking. Cancellations or rescheduling require 48 hours’ notice via the booking platform. Late cancellations incur a 25% fee. Disruptive behavior during group sessions may result in immediate removal without refund.

Beyond Genetics is not liable for any emotional, psychological, physical, or other outcomes from participating in group sessions. You participate at your own risk. This Agreement is governed by Florida law. By signing, you consent to electronic transactions under Florida’s Uniform Electronic Transactions Act (Fla. Stat. § 668.50).
